Eligibility for Care Memberships:

• Care Memberships are available only to established clients of Clover Basin Animal Hospital.

• A client is considered established after their first appointment at the hospital.

• A waiting period of 7 days applies after the first appointment before a Care Membership can be used.

Annual Agreement:

• The membership is a 12-month agreement that automatically renews unless canceled at least 14 business days before the renewal date.

• Each pet is enrolled individually, and the plan applies only to the enrolled pet.

Payment Terms:

• Payments can be made annually or monthly.

• Monthly payments are due on the same day each month, and failure to pay may result in suspension of services.

• If payments are not made, the provider can send unpaid balances to collections.

Cancellation & Financial Responsibility:

• If canceled within the first 30 days and no services have been used, the member is eligible for a full refund.

• If canceled after 30 days or if services have been used, the member must pay the full retail cost of services received or the remaining plan balance, whichever is less.

• Failure to make payments will result in the account being sent to collections, and the member is responsible for collection fees, legal fees, and any additional costs incurred.
